I have worked in Health and Social care since 2000 across acute, long term and intermediate care settings. Since 2014 I specialised in clinical Frailty and utilising Comprehensive Geriatric Assessment to identify clients' needs. In 2016 I gained further Post graduate qualifications in Physical Assessment of the Adult from Buckinghamshire New University.
I have a passion for promoting well-being and enabling individuals reach their functional potential in the community. I have an extensive experience of frailty complex case management, mental capacity assessment and care home placements and reviews, continuing health care check list, rehabilitation, wheelchair and postural management, manual handling and specialist equipment and adaptations, and supporting hospital discharges. I work alongside a number of different specialists, consultants, community teams and care providers with a purpose of optimising patient care and enabling access to relevant services.
My home visit initial assessment includes appx 90-120min face to face visit, comprehensive consultation, functional and cognitive review, clinical observations, clinical advice and agreed care plan. I produce a written assessment report, jargon free and follow up telephone call. The cost of the initial assessment is £340 and subsequent follow up home visit sessions are £95 (appx 60min duration) with an email or telephone follow up.
All initial referral telephone call consultations (up to 30min) are free of charge.
I also offer pro bono work for patients who can not afford the OT fees and I offer discounted services for patients with limited financial resources (subject to capacity and eligibility).
