Zinka Zecevic's profile

Zinka Zecevic I am a qualified Occupational Therapist who graduated from London South Bank University and have trained and specialised within London NHS Trusts since 2004. I am also a certified MoCA Cognitive Assessment Rater (GBZECZI160051-03).

I have worked across health and social care services since 2000s, with experience spanning acute hospitals, long-term care, intermediate care, rehabilitation, and community-based support services. Since 2014, I have specialised in clinical frailty and the use of Comprehensive Geriatric Assessment to help identify individual needs and support safe, person-centred care planning. I have also undertaken postgraduate study in Frailty Management and, in 2016, completed postgraduate qualifications in Physical Assessment of the Adult at Buckinghamshire New University.

Alongside my clinical practice, I have a strong interest in research, higher education teaching, evidence-based practice, and service development within frailty, dementia, rehabilitation, and community care. I have contributed to multidisciplinary teaching, student supervision, professional presentations, and quality improvement initiatives, with a focus on promoting holistic, person-centred approaches for older adults with complex needs. I also have experience in designing and delivering higher education teaching, student supervision, and supporting the development of future occupational therapy and AHP professionals.

I am passionate about promoting wellbeing, maintaining independence, dignity, and helping individuals achieve their functional potential within their home and community environments.

I support older adults and families experiencing frailty, dementia, reduced mobility, complex health conditions, rehabilitation needs, and increasing care requirements. I have particular expertise in supporting individuals with complex frailty, dementia, and multifactorial functional decline within community settings.

I have extensive experience in:

• Frailty and complex case management

• Dementia and cognitive assessment

• Telecare and risk management

• Mental capacity assessment

• Rehabilitation and reablement

• Continuing Healthcare checklists

• Wheelchair and postural management

• Manual handling

• Specialist equipment and adaptations

• Hospital discharge support

• Care home placements and reviews

I work collaboratively with consultants, GPs, specialist services, community teams, carers, care providers, and families to help individuals access the most appropriate support, interventions, and services for their needs.

My initial home assessment includes an approximately 90–120 minute face-to-face home visit and comprehensive consultation. This includes a detailed functional assessment, cognitive review, clinical observations, discussion of current concerns and goals, assessment of mobility, transfers, activities of daily living, home environment, equipment needs, risks, and overall support requirements. Clinical advice and recommendations are provided during the visit, alongside collaborative care planning tailored to the individual’s needs and circumstances.

Following the assessment, I provide a clear, jargon-free written report summarising findings, recommendations, and agreed actions, together with follow-up telephone support where required.

Examples of support I may provide include helping individuals remain safely at home for longer, reducing falls and hospital admissions, supporting safer mobility and transfers, identifying appropriate equipment and adaptations, supporting dementia care planning, advising on care needs and rehabilitation, improving carer confidence, supporting hospital discharge planning, and helping families navigate complex health and social care systems during periods of change or crisis.

I aim to provide practical, holistic, and person-centred recommendations that promote safety, dignity, independence, and quality of life for both individuals and their families.

The cost of the initial assessment is £360. Follow-up home visit sessions are £155 (approximately 60 minutes duration) and include email or telephone follow-up support. Urgent visits within 24–72 hours, subject to capacity, are available for an additional fee. Please contact me directly for further information. Initial referral telephone consultations of up to 30 minutes are provided free of charge. I am committed to making occupational therapy support accessible and may offer discounted or pro bono services for individuals experiencing financial hardship, subject to eligibility and availability.
