I am an independent occupational therapist, skilled in the assessment of neurological injury/illness and/or physical disability and the impact on adults' activities of daily living. My 20 years + experience in the NHS, military, higher education and charitable organisations has supported me in communication and joint working with relevant stakeholders including funders, care providers and families. I create and implement individual intervention plans within the home, community and work/study environment to support independence for the client and their carers. I can provide education of relevant stakeholders including care providers, families and the individual on the impact of the neurological and/or physical dysfunction on independence and offer skilled report writing for diverse audiences. I am able to provide services within the Buckinghamshire, Oxfordshire, Cambridgeshire, Berkshire, Bedfordshire, Hertfordshire, mid and South Essex and London areas. Services offered include: - Functional and cognitive assessments - Environmental assessment and recommendations - Assessment and recommendations around carer ability and needs - Vocational assessment and recommendations - Client centered goal planning - Creation and implementation of personalised treatment plans - Education to support key stakeholders around disability management - Report writing for stakeholders including; funders, families, the Court of Protection and relevant professionals.
