I offer a person-centred, client-led occupational therapy and case management service for adults 18 years old and over with moderate to severe symptoms of depression and anxiety, and long-term health conditions.
My case management service is based on the clinical case management model, which is combining therapy and case management approaches to support clients. I combine cognitive behavioural therapy (CBT) and occupational therapy techniques with case management to support clients achieve their personal goals.
I offer a comprehensive holistic initial assessment and report, and progress reports for clients that opt for case management support.
I have completed training in mental health crisis planning, suicide prevention, and Level 2 accreditation in CBT Essentials from the Association of Psychological Therapies. And I am MoCA certified to help identify clients with mild cognitive dysfunction receive early intervention.
