I have gained experience in complex case management, rehabilitation in homes, and worked in the Rapid Response and Discharge to Assess pathway to support hospital discharges. I have worked as part of multidisciplinary teams to manage complex cases, supporting patients with specific health needs which often require specialist equipment in order for them to remain safe in their home environment, and I provided end of life palliative care. I promoted health and wellbeing in order to maximise patients safety and empower their families
I have a lifelong passion for working with patients and their families in the community to promote independence and minimise risk of readmissions. I work extensively with patients� families to help them establish a support network and build up the knowledge about specialist services available in their area.
A home visit will be required to a complete a face-to-face core assessment, which all family members are welcome to attend. After the assessment I will complete a report that contains the outcomes and recommendations. Follow-up is provided via telephone or face-to-face reviews depending on what the situation requires. The initial telephone call consultations are free of charge. The cost of intervention will be agreed during the initial telephone consultation.
