I have knowledge of a wide range of long term conditions, co-morbid conditions and complications that may arise. I assess people in their own homes, residential settings and housing associations for housing needs, issues and equipment. I ascertain people's outcomes at assessment to support and encourage independence, choice and control to live purposeful and meaningful lives. I make recommendations for minor and major adaptations (such as wheelchair access, level access shower, kitchen and extensions). I can recommend equipment and provide moving and handling risk assessment and care plans. I have extensive knowledge of a wide variety of equipment as well as specialist bespoke equipment. I undertake seating assessments for postural and pressure relief needs and I work closely with the wider MDT, which includes social workers, health staff and care agencies.
