She aims to maximize the individuals independence in areas of daily living and in completes Standard or Complex assessments at home or in the community resulting in written reports and giving recommendations regarding equipment provision, minor and major adaptations, Disabled Facility Grant application, Care packages, Disabled Car Badge appeals and appropriate sign posting to other agencies. She covers the geographic area of North Wales, Cheshire, Warrington and surrounding areas
