A review of your home environment to ensure it remains as safe as possible for you.
Washing and dressing and all aspects of personal care.
Cooking and kitchen activities.
Accessing the local community.
Engaging in social activities including assessment using the Pool Activity Level (PAL).
Life story work, validation therapy and reminiscence.
Advice on benefits eligibility and support in applying for them.
Support with facilitating prompt, safe and appropriate hospital discharges through the organisation of care or identification of suitable convalescence homes.
Organising appropriate packages of care using reputable care providers.
Practical support and advice when considering a move into a care home, including identifying the right care home for you.
Monitoring of care to ensure it remains appropriate to your needs over time.
Mental Capacity Assessments.
Linking you in with support services as appropriate, such as Social Services, Age UK, Carers Support, Alzheimers Society.
End of life planning.
