Moving Together Devon CIC is the culmination of my two working passions spanning over thirty-five years as an Occupational Therapist specialising in the Social Care and Housing sector, many of these years as an independent practitioner. Over the last five years I have worked as a community Occupational Therapist and care manager working with people with complex issues. I have also managed housing Occupational Therapists as part of a Home Improvement Team, working collaboratively with surveyors and technicians. As an independent practitioner I have covered a wide range of case work. Here are some examples over the past year: A Treating Occupational Therapist for a case management team, using a solution focused approach, part of which was identifying equipment and adaptations. Enabling safe discharge from residential care, back home, through close liaison with the home, care services, family and the provision of equipment Follow up after recent hospital discharge with provision of equipment and minor adaptations Recommending short, medium and long-term solutions to enable someone with a progressive disability to consider what type of home modifications would be suitable for their future. One-off assessments to provide recommendations regarding specific activities. Sensitively helping a couple move from a 4 bed townhouse to a two bed retirement home, including: finding a new home, sorting and clearing belongings, buying specialist equipment and support with moving in.
